Assess the Elder&rsquo;s needs in accordance with the&nbsp;InterRAI&nbsp;tool and provide a plan of care and interventions based on an evaluation of outcomes.</p> <p><strong><em>Duties and Responsibilities</em><br /></strong>Administration of

  • Treatments:</p> <ul> <li>Administer and document medications and treatments for all Elders, and assess effectiveness</li> <li>Report Elder response to medication and treatments to physician/nurse practitioner</li> <li>Transcribe and process all orders received</li> </ul> <p>Communication and
  • Documentation:</p> <ul> <li>Chart care provided, behaviours and incidents</li> <li>Respond to Elder and family inquiries</li> <li>Educate Elders, families, and staff</li> <li>Complete&nbsp;InterRAI assessments, CAPs and care plans according to schedule</li> <li>Act as a mentor for students during clinical placement</li> <li>Provide guidance and direction to Personal Support Workers, and other staff working on the assigned unit</li> </ul> <p>Elder
  • Care:</p> <ul> <li>Assist as necessary with Elder care and transferring</li> <li>Ensure Elder and family are comfortable during palliative phase</li> <li>Monitor levels of liquid oxygen in portable tanks and fill as necessary</li> </ul> <p>General:</p> <ul> <li>Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed</li> <li>Incumbents must possess the physical ability to meet the duties and responsibilities of the job description and/or the requirements identified within the position's Physical Demands Analysis&nbsp;</li> </ul> <p>These describe the general nature and level of work being performed by incumbents in this classification.
