Job summary

The Family Counsellor / Social Worker is responsible for providing culturally safe, trauma‑informed counselling services to children, youth, and families. The Family Counsellor develops and implements a family‑focused social work framework within the Child and Youth Health Services team, supporting individuals and families in their wellness journeys.

Position description –child and youth health services

Position Title: Registered Social Worker – Family Counsellor

Reporting relationship

Reports to and works under the direction and supervision of the Maternal Child and Youth Health Manager, Health Services, Six Nations Elected Council.

Purpose & scope of the position

The Family Counsellor / Social Worker is responsible for providing culturally safe, trauma‑informed counselling services to children, youth, and families. The role focuses on strengthening family relationships, improving communication, supporting healthy parenting practices, and promoting wellness and resilience through therapeutic family centered interventions.

The Family Counsellor develops and implements a family‑focused social work framework within the Child and Youth Health Services team, supporting individuals and families in their wellness journeys.

Key duties & responsibilities

1. Technical Functions

• Conduct comprehensive psychosocial and family‑systems assessments, intake, and ongoing evaluation
• Provide family, individual, and parent–child counselling using evidence‑based and culturally grounded approaches
• Deliver therapeutic interventions related to family conflict, parenting challenges, trauma, grief and loss, separation/divorce, anxiety, depression, attachment, and behavioural concerns
• Utilize trauma‑informed, strengths‑based, and culturally safe practices in treatment planning and service delivery
• Develop, implement, and document individualized and family‑oriented treatment plans
• Provide crisis intervention, suicide and risk assessment, and safety planning as required
• Offer counselling and education to parents/caregivers to enhance parenting capacity and family functioning
• Facilitate family reunification, mediation, and relational repair where appropriate
• Deliver group programs and workshops focused on parenting, family wellness, coping skills, and resilience
• Implement the Haudenosaunee Wellness Model, using culture as the foundation of counselling services
• Deliver land‑based family and youth programming
• Provide systems navigation, advocacy, and referrals to internal and external resources in collaboration with case managers
• Participate in accreditation‑related activities as required

1. Communications Functions

• Communicate effectively and respectfully with clients, families, community members, and interdisciplinary team members
• Advocate for children, youth, and families while maintaining professional boundaries
• Attend case conferences, clinical rounds, and team meetings, providing a family‑counselling perspective
• Establish collaborative working relationships with supervisors, clinical leads, and partner agencies
• Maintain confidentiality and privacy in accordance with PHIPA and organizational policies

1. Administrative Functions

• Maintain accurate, timely, and professional clinical documentation within the Electronic Medical Record
• Ensure records meet Ontario College of Social Workers and Social Service Workers (OCSWSSW) standards
• Complete administrative and reporting requirements as directed by the Manager
• Contribute to work plans, month‑end, and quarterly reports
• Participate in ongoing professional development and training relevant to family counselling and scope of practice

1. Other Functions

• Performs other job related duties as may reasonably be required by the Child & Youth Health Team Manager
• Provides supervision of students and volunteers
• Follows established Ontario College of Social Workers and Social Service Workers guidelines for the supervision of students
• Provides direct on-site supervision of the student/volunteer as stipulated in the supervision guidelines
• Obtain appropriate consent before allowing a student/volunteer to participate in treatment for an individual
• Provides support to the Six Nations Emergency Measures Plan by ensuring awareness of the Emergency Measures Plan and assistance as instructed by the Director of Health Services
• It is the responsibility of the employee to be aware of and adhere to all policies and procedures, including those that relate to client safety, staff safety and risk management

Knowledge and skills

Minimum Requirements

• Master’s Degree in Social Work, RSW from an accredited Social Work Program

OR

• Bachelor’s Degree in Social Work, RSW from an accredited Social Work Program plus 3-5 years of counselling experience
• Must be a member in good standing and hold current membership with the Ontario College of Social Workers and Social Service Workers.
• Well-developed counselling skills using a culturally safe, trauma-informed lens
• Demonstrated knowledge and understanding of counselling interventions and clinical practice with children and youth
• Strong interpersonal, verbal, and written communication skills
• Understands importance of confidentiality under PHIPA
• Must be well-versed in relevant consent and capacity legislation such as Social Work Code of Ethics, Mental Health legislation and the Health Care Consent Act.
• High level of computer skills of Microsoft Office, MS Word, Excel and working knowledge and experience with electronic medical records.
• Preferred experience working in child/youth health or a related child/youth service field
• Valid Class “G” license and reliable transportation

Other Related Skills

• Familiar with the Six Nations Community and highly sensitive to Six Nations Culture
• Knowledge of family systems theory, attachment‑based practice, and evidence‑based family counselling approaches.
• Exhibits a high degree of initiative and self-direction
• Ability to work with tact and discretion; high level of public relation skills and good project management skills.
• CPR and First Aid certification preferred
• Highly motivated, able to work independently with minimal supervision.
• Family therapy training (CBT, EFT, attachment‑based, etc.)

Impact of error

Errors in judgement and in the conduct of duties could lead to loss credibility, poor public relation, confusion, duplication of effort and misinformation being given to the Director of Health Services, Human Services Committee, Six Nations Elected Council, Government Agencies and the public.

CONTROL
