Support and embrace resident-centred social culture and deliver high-quality personal care.
Work as a key member of the Village and Neighbourhood multidisciplinary team to understand residents’ individual needs and advocate for residents’ participation in decisions about their care.
Support new resident move-ins, facilitate transfers to hospital, and respond to changes in residents’ medical needs.
Collaborate with the Neighbourhood Physician during weekly rounds and update care plans and directions accordingly.
