About the role

Deliver comprehensive care to residents within an interdisciplinary team, including health assessments, diagnoses, management, evaluation of care outcomes, performing procedures, and counseling.

Identify and implement research-based innovations to improve resident care at individual, organizational, and system levels.

Requirements

Current registration as a Nurse Practitioner with the College of Nurses of Ontario.

Experience completing comprehensive geriatric assessments.

Current Heart Save Level of CPR.

Maintain a current AIS certification.

Experience leading, coaching and mentoring care professionals in a healthcare setting.

Knowledge of relevant policies, procedures, legislation and regulations applicable to the long-term care sector, including the Long-Term Care Homes Act, 2007 and Health & Safety.
