About the role

The Health Navigator coordinates resident assessments, care plans, and wellness services while collaborating with families and the care team. They also provide direct nursing care, medication administration, and ensure regulatory compliance within the retirement community.

The Opportunity

Are you passionate about helping seniors navigate their health journey with confidence and compassion? As a Health Navigator at Chartwell, you'll work closely with the Health & Wellness Manager to coordinate resident assessments, care plans, and wellness services. You'll collaborate with residents, families, physicians, and the care team to ensure every resident receives personalized, high-quality care in a welcoming retirement community.

  • What You'll Do
  • Complete resident assessments and develop individualized care and service plans
  • Coordinate resident admissions, care conferences, transfers, and discharges
  • Provide nursing care, medication administration, and resident education as required
  • Partner with physicians, families, and the care team to support changing resident needs
  • Maintain accurate clinical documentation and monitor resident wellness outcomes
  • Support infection prevention, quality improvement, and regulatory compliance initiatives
  • What You'll Bring
  • Registered Practical Nurse (RPN/LPN) or Registered Nurse (RN) in good standing with the provincial College of Nurses
  • Current CPR/First Aid certification and medication administration knowledge
  • Experience in retirement living, long-term care, assisted living, or a related healthcare setting
  • Strong assessment, clinical, organizational, and communication skills
  • Compassionate, collaborative, and resident-focused approach to care
  • Experience working with electronic health records and Microsoft Office

Requirements

Candidates must be a Registered Practical Nurse (RPN/LPN) or Registered Nurse (RN) in good standing with the provincial College of Nurses. Experience in retirement living, long-term care, or assisted living settings is required, along with current CPR/First Aid certification.
