Provide direct nursing care and professional support to participants in the qathet Complex Care Housing program.
Assess health needs and contribute to participant stability and recovery.
Deliver trauma-informed interventions.
Support harm reduction and relapse prevention.
Collaborate with interdisciplinary teams, families, and community partners to improve housing and health outcomes.
Provide withdrawal management and stabilization support.
Administer and monitor medications.
Observe symptoms/behaviours and escalate/report as needed.
Perform crisis intervention and de-escalation using trauma-informed practice.
Provide recovery-focused education/counselling.
Support case management and transitions of care between program and community services.
Maintain documentation and health information management (charts, stats, reports).
Support safety and risk management (violence prevention, infection control, OH&S).
Participate in case conferences and treatment planning.
Contribute to quality improvement and continuous learning
