The Case Manager provides client-centered support and coordinates care for individuals experiencing homelessness, mental health challenges, and substance use. Key duties include managing a caseload, conducting assessments, facilitating wellness groups, and assisting clients in accessing housing and healthcare resources.
This position is part of the CUPE [Local # 2289-05] bargaining unit and is subject to the terms and conditions outlined in the collective agreement.

The Case Manager provides client-centered support to individuals experiencing homelessness, substance use, mental health challenges, and complex social needs. Working as part of a multidisciplinary team, the Case Manager is responsible for coordinating care, supporting access to services, and helping clients develop and achieve their goals through individualized service planning.
