Follow client care plans and deliver high quality client care.
Prepare meals and snacks according to the meal plan.
Use the Catalyst system to assist with medications on time per Excel policies and procedures, including checking/receiving medications arriving while on shift.
Perform general household duties and maintenance (cleaning, vacuuming, laundry) and outside maintenance (shovelling snow, cutting grass, gardening).
Maintain and update required documentation (scorecards, medical records, etc.).
Assist clients with hygiene and personal care.
Provide emotional support and use de-escalation techniques.
Support clients in maintaining appointments, activities, or programs.
Monitor client wellbeing and physical/mental condition and report concerns to the supervisor; accompany clients to appointments as necessary.
