The In-Patient Palliative Care Unit at Trillium Health Partners (THP), located at the Mississauga Hospital (MH) site, consists of 16 beds dedicated to providing specialized acute palliative and end of life care.
Working in close collaboration with palliative care physicians and an interprofessional team, the Registered Nurse (RN) plays a key role in assessing and addressing a broad range of patient needs. This includes symptom management, medication concerns, psychosocial and caregiver challenges, functional changes, goals-of-care planning, and community care transitions. The RN applies specialized palliative care knowledge, critical thinking, and sound clinical judgment to identify patient needs, provides emotional support, safe and effective care planning, and escalate urgent clinical issues when necessary.
