Provide psychosocial assessments, individual and family psychotherapy, and facilitate psycho-educational groups for geriatric mental health patients. Coordinate patient care within the hospital and collaborate with community resources for effective discharge planning.
Requires a Master of Social Work and registration in good standing with the O.C.S.W.S.S.W. Candidates should have 3-5 years of mental health social work experience, preferably with geriatric populations and psychotherapy expertise.
In a direct social work practice, some of the job responsibilities the social worker in this position will provide include: psychosocial assessments for individuals referred from within the hospital as well as from the community; individual psychotherapy, marital and family therapy, plan and conduct psycho educational and process groups, facilitate an evening family group, consult and co-ordinate patient care within the hospital as well as collaborate with community resources, complete forms and letters as required for the patient care and discharge planning, follow the documentation guidelines as defined by the social work college as well as the mental health department, and participate in ongoing professional development.
Masters of Social Work Registration with the O.C.S.W.S.S.W. and in good standing Minimum of 3-5 years social work experience in Mental Health Experience working with geriatric patients an asset Experience in developing and conducting psychosocial and process groups Demonstrated experience in providing psychotherapy including family therapy Excellent psychosocial assessment and intervention skills including crisis intervention Demonstrated understanding of serious & persistent mental illness, concurrent disorders and its presentation in the geriatric population Familiarity with community resources and demonstrated ability to work well with community partners Experience in the treatment of mood and anxiety disorders and other complex mental health issues Demonstrated excellent organizational, interpersonal and communication skills. Must be able to work independently and interdependently within multidisciplinary treatment team Computer proficiency with Microsoft Word, PowerPoint, Outlook, Meditech, and Epic Excellent attendance, punctuality and work record
