We are hiring a registered Social Worker for the Lillooet Hospital and Health Centre. Days of work: Monday to Friday Shift times: 08:00 to 16:00

About this position: This is a home health social work position supporting. Local travel is required within this role in the surrounding area. Access to personal vehicle will be required; a valid Driver’s License is necessary. In this role, you’ll provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, you deliver direct psychosocial care, including counseling and group work, while supporting system navigation and advocacy. You also provide education, connect clients to relevant specialized and complex care resources, and facilitate client self-management. You are responsible for delivering and advancing social work services that maintain or improve the health, well-being, and overall functioning of the identified population. Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience. Current full registration with the British Columbia College of Social Workers. Current valid BC driver’s license.
