What Are We Looking For?

Position Title

Case Manager, Housing With Recovery Supports (HRS) Job Type Full-time, permanent Department Housing Reports to Team Lead, HRS

Position Overview We are seeking a passionate and dedicated Case Manager to join the Housing with Recovery Supports team at Inn from the Cold. The Case Manager, HRS works with a caseload of 15 families housed either in the community or at Neoma; families generally remain in the program for up to two years. They will provide intensive case management supports, assisting families from initial sourcing of housing, through development of case plans to work towards addressing short and longer term needs and goals, to sustainable independence in the community.

  • What You’ll Do Support participant families in sourcing housing – either in designated units at the Neoma building or in the community.
  • Support participants in developing a strength-based case and participant-led case plan to address needs and to attain goals over time.
  • Meet with families frequently (in-home and in the community) to provide support and monitor progress.

  • Provide ongoing advocacy & resource navigation support to address barriers and assist families in accessing culturally relevant community resources, employment services, mental health support, health care and financial assistance.
  • Review and refine goals with participants regularly, utilizing assessment tools (Outcome Star and Service Plan).
  • Support participants as their goals and actions shift, crisis or significant change is experienced.
  • Maintain accurate, timely, and confidential documentation of family interactions, case notes, intake and assessment information, safety concerns and records of support and service referrals using HOMES and HMIS databases, physical participant file.
  • Work both physically in the office, and in the community – frequent travel within the city may be required.
  • This position description reflects the general duties of the role, but is not intended to include all duties that may be inherent in the position. The Case Manager, HRS may be asked to perform other duties as required or assigned.

    • What You Bring to Our Mission Completion of a diploma or degree program related to social work, family case management, or human services is preferred but a combination of education, experience and training will be considered.
    • 2 years experience in family case management, family support and/or crisis support.
    • Experience working with vulnerable populations experiencing homelessness.
    • Knowledge of trauma-informed and participant-centered care and conflict resolution skills.
    • Knowledge and understanding of issues related to homelessness and its impact on families Computer literacy skills are required, including the ability to use MS Office 365 and Teams.
    • Excellent communication, problem-solving, and teamwork skills.
    • Able to communicate effectively both verbally and in writing in the English language.
    • Able to work effectively and safely without direct support or supervision in the community.
    • Work shifts will generally be Monday-Friday business hours; work In the evening/on weekends may be required from time-to-time.
    • Current First Aid/CPR Level C is required, Suicide Intervention (ASIST) and Crisis De-escalation training is an asset.
    • Police check and Child Intervention check are required for employment.
    • A valid driver’s license (class 5) and reliable vehicle are required.
