Requirements summary

  • Recent experience providing support to individuals with serious and persistent mental health issues OR considerable relevant and recent experience in outreach and support work with marginalized populations.
  • Experience supporting the planning and carrying out of client care plans, including daily routines and Activities of Daily Living (ADL).
  • Education or training in social services, social work, psychology, or other relevant areas (asset).
  • Experience supporting individuals in crisis or exhibiting challenging behaviours (asset).

Who We Are

  • The Government of Yukon's Mental Wellness and Substance Use Services division is known for its innovative, holistic, trauma informed and culturally accessible approach to providing mental health and substance use supports to all Yukoners. As a valued member of our Psychiatric Outreach Program and Early Psychosis Intervention (POP EPI) team, you will play a pivotal role in supporting individuals navigating serious and persistent mental health concerns, including bipolar disorder, schizophrenia, and other psychosis-related conditions. Your role The Mental Health Support Worker is responsible for working as part of the Psychiatric Outreach Program in delivering 1 to 1 supported independent living services to clients. The incumbent assists with planning and implementing psychosocial rehabilitation groups and working closely with other community agencies as resources in the provision and coordination of these services to enable clients to live as independently as possible in the community. The position functions as part of a multidisciplinary team in an outreach capacity, supporting clients in achieving their treatment goals in collaboration with their clinician. The ideal candidate is passionate about working with individuals experiencing serious and persistent mental illness and brings experience supporting clients in community and home-based settings. They are able to work independently, while also contributing positively to a collaborative team environment and adhering to established workflows.

    • Exposure to clients who may engage in unpredictable behaviour and may pose a risk to themselves or others
    • Exposure to individuals experiencing mental health challenges, emotional distress, or deteriorating mental status
    • Interaction with clients who may display aggressive behaviours related to illness and/or substance use
    • Working in a variety of community settings, including conducting outreach in clients’ homes and other environments, which may present physical barriers (e.g., stairs)
    • Exposure to unsanitary or potentially infectious environments in community or home settings
    • Potential exposure to second-hand smoke
    • Regular travel within the community (approximately 20–120 km daily), often transporting clients
    • Required to travel during all weather and road conditions, particularly during winter months
