Work as a key member of the multidisciplinary team to understand residents’ needs and advocate for residents’ participation in care decisions.
Support new resident move-ins and facilitate transfers to hospital or respond to changes in residents’ medical needs.
Collaborate with the Neighbourhood Physician during weekly rounds and update care plans and directions accordingly.
Use proficient communication and computer skills to update care plans in the current system.
