Provide clinical care, illness prevention, and health promotion using a client-centred and interdisciplinary team approach. Identify physical, mental, and psychosocial needs of clients within a community health centre environment.
Requires current College of Nurses of Ontario registration as an RN or RPN with 3-5 years of nursing experience. Must possess a valid Ontario driver's license, access to a vehicle, and the ability to work flexible hours.
RN/RPN with current College of Nurses of Ontario registration. Minimum 3-5 years nursing experience. Ability to work flexible hours including evenings and weekends. Strong organizational, communication and leadership skills. Ability to work both independently, and collaboratively, within interdisciplinary team environment. Perform clinical care according to the College of Nurses standards for nursing practice. Utilizes professional assessment skills to identify physical, mental, and psychosocial needs of clients. Knowledge and proficiency in current evidence based practices of primary care delivery, with an emphasis on health promotion and harm reduction. Proficiency in electronic charting. Has awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured). Valid Ontario driver’s license and access to a vehicle.
