Provide comprehensive case management for clients with severe mental illness who are homeless or transitioning to independent living. Responsibilities include conducting psychosocial assessments, monitoring wellness, and collaborating with treatment teams and housing providers.
Requires a professional qualification in a health discipline and over 5 years of experience with adult clients suffering from serious mental illness. Candidates must possess a valid driver's license, own vehicle, and proven skills in crisis intervention and risk assessment.

To provide case management services for a caseload of clients with severe mental illness who are homeless, at risk of homelessness or transition to independent living in the Mississauga/South Etobicoke area. Case Managers work in partnership with clients to develop the skills and supports they need to live successfully in the community. Clients are supported through visits in their own home, in the community or in the office.
