Provide person-centered outreach support to clients to enable their successful adjustment to community life. Develop individualized goal plans and assist clients in accessing housing, employment, and essential community services.
A diploma in Social Services and one year of relevant experience are preferred, along with a valid driver's license and a reliable vehicle. Candidates must pass a Vulnerable Persons Criminal Records Check and hold a current Emergency First Aid certificate.
KEY ACCOUNTABILITIES Works with the client, researches background material, and seeks the input of other professionals and family members, to assess needs and intervention possibilities Works with the client to consider issues and agree on an intervention and support plan with measurable short and long-term goals Ensures the client’s intervention and support plan addresses issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure, lifestyle, access, employment, and education Supports clients in various environments, which may include exposure to bedbugs, animals, needles, blood, lice, human waste, hording and other Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long-term goals
EXPERIENCE AND SKILLS A Diploma in Social Services and one-year relevant experience is preferred; inhouse training can be provided for the right candidate Professional or lived experience working with vulnerable populations experiencing multiple barriers A demonstrated and clear ability to respond well in crisis situations
PHYSICAL REQUIREMENTS The physical requirements outlined below are non-specific to the position, as such, there may be additional tasks required. Ability to walk up and down stairs Ability to operate a motor vehicle Ability to move items weighing up to 50 pounds Ability to perform First Aid, CPR, and Naloxone administration Ability to remain in a stationary position for extended periods of time Ability to complete general housekeeping duties (i.e., sweeping, mopping, wiping areas, laundry, changing light bulbs)
Ability to successfully complete a Vulnerable Persons Criminal Records Check Ability to successfully pass a reference check Current Emergency First Aid Certificate (Please refer to the Certificates accepted in B.C. - WorkSafeBC). Proof of vaccination for communicable diseases may be required as mandated by Government, Health Authority, and/or funder requirements Valid driver’s license Personal transportation that is in good repair, insured for business up to $1,000,000 for liability, and which can be used to transport clients
COMPENSATION As Connective is a unionized environment, the starting wage is $27.54 per hour plus 10.6% in lieu of vacation pay and benefits (for casual employees). The wage will increase with every 2,000 hours worked up to a maximum of $31.61 per hour.
Candidates must be legally authorized to work in Canada, observe their weekly hours limit if under a temporary or study visa, and provide proof of eligibility if selected for the role.
