You will collaborate and communicate closely with a multi-disciplinary team to help clients achieve positive outcomes. You will support with administrative tasks, including scheduling appointments and communicating with third-party funders. You’ll work in a fast-paced environment as an integral member of a dynamic multidisciplinary team, while sharing our core values of communication, community, excellence, health, and innovation. Accountabilities Schedule and complete orientation sessions for clients when initiating treatment and ensure smooth transition of clients into program. Provide thorough assessment and exercise prescription for clients within a holistic approach. Support intake coordinator in managing WCB waitlist and provide information regarding program. Design rehabilitation plans for clients with physical injuries with the goal of returning client to pre-accident level. Complete functional testing evaluation and re-assessments as indicated. Assist in providing ongoing case management and client support. Facilitate one to one and group exercise and pain education sessions. Participate and facilitate weekly case consultations with the interdisciplinary team. Oversee client care, communicating progress, concerns, and outcomes with other clinicians. Communicate and collaborate with team members to ensure treatment plans are aligned. Ensure same day completion of session notes. Template and complete initial, progress, and discharge reports for funders. Ensure these reports are submitted in a timely manner. Maintain regular communication with the claim owner or case manager for client’s progress in programming, scheduling of case conferences and extension requests etc. Monitor schedule and communicate with administrative team to ensure clients are booked in appropriate sessions and appointments. Other responsibilities related to the role and scope of work may be assigned. WHAT YOU BRING
