The Registered Dietitian assesses nutritional needs and develops individualized care plans for older adult clients. They collaborate with a multidisciplinary healthcare team to provide evidence-based dietary counseling and health teaching.
Candidates must hold a Bachelor's degree in Applied Human Nutrition or Science and be registered with the College of Dietitians of Ontario. A valid driver's license, reliable vehicle, and current CPR/First Aid certification are also required.
Provide nutrition service to clients that incorporates all components of the therapy process model Work as part of a multidisciplinary team to deliver excellent client outcomes Provide assessment and treatment virtually or in-person as required to a wide variety of older adult clients with varying needs including malnutrition, chronic disease management, enteral nutrition, texture modified diets etc. Provision of health teaching, hand-outs, and consultation to patient and caregivers, including potential residential home staff Work closely with the team and other resources to develop new programs to meet identified client needs Perform regular, timely charting, reporting and communication Record workload measurement data Attend staff meetings and case conference meetings, as required Comply with continuous quality improvement initiatives of Bayshore Home Health Developing a flexible, responsive system to meet changing and competing demands
