Physician/Hospitalist (Locum), Care of the Elderly

Bruyère Health, an Academic Health Science Centre, excels in the provision of evidence-based health care and services for the vulnerable and medically complex, with a focus on persons who require sub-acute, geriatric, stroke or palliative care. Among other programs, Bruyère Health provides in-patient and out-patient clinical services in Care of the Elderly, Complex Continuing Care, Palliative Care, Family Medicine and Physical Medicine and Rehabilitation. Bruyère is one of the largest health care centers of its kind in Canada and a resource for information on the special care required by the elderly, people with chronic and terminal illnesses, and persons having suffered loss of autonomy.

The Care of the Elderly Partnership at Bruyere is currently looking for LOCUM family physician hospitalist coverage on Bruyère's inpatient Geriatric Rehabilitation Service. There are opportunities for short term and long-term locums.

Work Expectations

• The services of a locum hospitalist physician with an interest in the interprofessional care of older adults living with frailty are needed on the Geriatric Rehabilitation Service. The provider will be joining a large, collegial and supportive group to ensure the general medical care of patients admitted to this high-intensity rehabilitation program, with responsibilities including:
• Admission assessments integrating a comprehensive geriatric approach
• Ongoing daily medical care, e.g. including diabetes management, hypertension, and infections
• Regularly collaborating with other interprofessional team members
• Actively contributing to weekly Interprofessional Team Rounds
• Completing weekly medication reviews with the Pharmacist to optimize prescribing
• Actively contributing to family conferences as needed
• Preparing discharge prescriptions and completing discharge summaries on day of discharge
• Daytime availability for regular visits and to manage emergency medical situations from 8AM to 5PM weekdays (no need to be in person the full day, but availability over the phone is required)
• Evening and weekend call, shared with a roster of 10-12 physicians

Remuneration

• Competitive pay
• Funded through an Alternate Funding Plan

• Reporting directly to the Chief of the Care of Elderly Department, the successful candidate will have:
• Doctor of Medicine (MD) Degree
• Appropriate Certificate in medical practice from the College of Physicians and Surgeons of Ontario (CPSO) and the College of Family Physicians of Canada
• Registration in good standing with the CMPA
• Excellent interpersonal and organizational skills
• Previous experience/demonstrated interest in working with this patient population.
• Bilingualism (French/English) would be an asset.
