The Nurse Practitioner will provide comprehensive clinical care, health assessments, and diagnosis for clients within community and mobile settings. They will also collaborate with an interprofessional team to deliver culturally safe, preventive care and support population health initiatives.
Candidates must hold a Baccalaureate in nursing and a current Certificate of Competence with the College of Nurses of Ontario. A minimum of 3-5 years of related clinical experience and the ability to travel to community outreach locations are required.
Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.
With a staff of more than 550 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are “home-bound” and frail.
We are currently seeking a Nurse Practitioner to fill a vacant position at our Community Health team.
The Carefirst Community Health Team supports population health by bringing preventive care, health education, screening, and system navigation directly into the community, with a focus on equity and early intervention. By working alongside community partners, the team helps identify unmet needs, reduce barriers to care, and connect individuals to the right health and social supports at the right time.
These programs aim to improve equitable access to care, promote wellness, and strengthen connections within diverse communities.
