The Nurse Practitioner will provide medical care, symptom management, and end-of-life guidance to patients through home and virtual visits. They will also collaborate with an interdisciplinary team to ensure continuity of care and participate in an on-call rotation.
Candidates must have 3-5 years of experience in Hospice/Palliative care and be licensed with the College of Registered Nurses of Alberta. A valid driver's license and access to a vehicle are required for community travel.
Nurse Practitioner (Temporary Part-Time)
1-year term opportunity with possibility of extension
Reporting to the Medical Director, the Nurse Practitioner (NP) will be responsible for providing medical care to clients in the Home Visiting Hub (HVH) program. The NP will liaise with the HVH Manager, the HVH nurses and HVH physicians supporting continuity of care and 24/7 prescriber coverage. The NP is supported by the Home Visiting Hub Manager and nursing staff.
The NP will provide home visits to 15-20 clients in the community who are unattached and require palliative care. NP’s are expected to be available to HVH patients, for in-person and virtual visits, Monday through Friday, 8am-4pm, adjusted accordingly based on FTE.
