The Licensed Practical Nurse will conduct scheduled community visits to provide ethical and safe nursing care to patients of all ages. They are responsible for assessing patient needs, executing care plans, and maintaining accurate professional records.
Candidates must hold an active registration as a Licensed Practical Nurse with the CLHA and possess a valid driver's license with access to a vehicle. Experience in community care and working with vulnerable populations is considered an asset.
In this community nursing role, you as a Licensed Practical Nurse (LPN) will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.
