The LPN collaborates with a multidisciplinary team to assess, plan, and implement care for residents with established health needs. Key duties include administering medications, supervising HCAs, and maintaining up-to-date resident care plans using MDS-RAI.
Candidates must hold a valid CLPNA license and a CPR certificate. Experience in geriatric, rehabilitation, or long-term care nursing is preferred, along with a required criminal record check and proof of immunizations.

The Licensed Practical Nurse works as a member of the multidisciplinary team, collaborating on the delivery of care to meet the needs of residents. The LPN provides care to residents whose health condition and needs are well defined and established, predictable with little fluctuation over time. The LPN assesses, plans, implements, and evaluates the care of our residents and works in consultation with HCA and RN colleagues and others to meet the needs of more complex patients.

Monitor and establish standards of care and practice to provide an environment that facilitates quality resident care delivery Monitor residents’ health, note change in condition and assess need for referral to physicians Process and implement physicians’ orders Collaborate with the multidisciplinary team to develop and maintain up-to date resident Care Plans to meet therapeutic, restorative and rehabilitative goals of each resident utilizing MDS-RAI Supervise nursing care Complete daily audits on Point of Care (POC) documentation Administer medications according to administrative and nursing policy Supervise the HCAs in assisting with medications Evaluate and document effectiveness of all medications Collaborate with pharmacists Observe, report, and document changes in resident status and deviations from anticipated resident responses/outcomes according to policy/charting guidelines and standards Notify residents’ families or next of kin of any changers to residents’ health status
In accordance with Alberta's Protection for Persons in Care Act, a criminal record check is required for successful candidates.
As a condition of employment with “BSF”, all new Health Care Worker hires must provide written proof of immunizations for communicable diseases.
