The Foot Care Practitioner provides routine foot care nursing services and performs assessments for residents in a long-term care setting. They also participate in multidisciplinary meetings to ensure care plans are followed and resident health changes are documented.
Candidates must hold an Advanced and Diabetic Foot Care Certification and be registered or eligible for registration with the Registered Practical Nurses Association of Ontario. A valid G class driver's license and access to reliable transportation are required for travel between sites.

Reports to: Operations Manager The AFCN RPN plays a pivotal role in enriching the lives of residents by supporting the LTC Home in the administration of treatments and assessments. The Foot Care Practitioner will have the opportunity to providing routine foot care nursing services to residents and clients within the portfolio. This position actively participates in all multidisciplinary meetings to ensure excellence in resident care and uphold the quality of nursing practice in accordance with: College of Nurses of Ontario Standards Long-Term Care Standards Southbridge Care Homes Standards
Ensure resident care is provide with established nursing standards and principles, physicians’ orders, and administrative policies, as well as observe, report, symptoms, and conditions of residents, and initiate remedial health measures where appropriate. Assure observance of required techniques in nursing care. Assess residents and contribute to resident care plans. Monitor residents’ health, notes changes in condition and assess need for referral to doctors. Perform foot care for procedures for which recognized training and current competency can be demonstrated. Observe and document client conditions and changes following proper standards and in accordance with professional standards. Qualifications. Advanced and Diabetic Foot Care Certification is required. Registration or eligible for registration with the Registered Practical Nurses Association of Ontario. Access to reliable transportation and “G” class license. Willingness to travel between work sites if required. Comprehensive knowledge of nursing and health care practices, as well as knowledge in Infection Control Practices. Effective human relation skills and demonstrated commitment to working with seniors. Effective communication skills and ability to work independently and collaboratively are essential. Additional Requirements include A negative vulnerable sector check completed within 6 months of start date. Completion of a 2 Step TB Skin test within the last 12 months of start date.
