The Nurse Practitioner will provide comprehensive primary health care to a registered client caseload and participate in the Community Diabetes Program. Responsibilities include clinical assessment, diagnosis, management of episodic and chronic illnesses, and collaborative work within an inter-disciplinary team.
Candidates must hold a BScN with a Primary Health Care Nurse Practitioner certificate and be registered with the College of Nurses of Ontario in the Extended Class. The role requires 2-5 years of broad-based nursing experience in a primary health care setting and the ability to work flexible hours.
Woolwich Community Health Centre (WCHC), is a multi-site health centre and requires a Nurse Practitioner (22.5 hours per week 0.6 FTE Permanent, at our Linwood site). All WCHC Nurse Practitioners work to their full scope of practice within an inter-disciplinary team to provide comprehensive primary health care to a registered client caseload. In addition to providing comprehensive primary care, within this unique role, the Nurse Practitioner will also engage in community-facing programing aimed at reducing primary care unattachment rates across rural Waterloo Region and surrounding areas. Woolwich Community Health Centre is responsible for program and service delivery to clients who are residents of Woolwich, Wellesley, and Wilmot Townships.
