Who We Are

CarePartners is one of Ontario’s largest accredited home health care providers, providing nursing, personal support, therapy and rehabilitation support services for patients of all ages. We care for approximately 72,000 patients each year in homes, schools, clinics, workplaces and retirement homes, through our 24 locations and 24 nursing clinics. CarePartners is proud of its commitment to quality, relentlessly seeking to improve the patient and caregiver experience, promoting a healthy and resilient workplace culture, and contributing to a sustainable healthcare system in Ontario. Overview CarePartners has embarked on a new program in partnership with the Ontario Health at Home (OH@Home) Central East (Durham Region) and we are looking for Community Support Workers (CSWs) to work within the High Intensity Supports at Home (HISH) program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.) are working cohesively to meet the needs of the patient. These PSWs will receive additional training and certification in Gentle Persuasive Approach (GPA). What Makes this Position so Exciting? In this exciting staffing model, HISH CSWs will work 12 hour shifts and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers. The hours of work will be 7, 12-hour shifts within a 2-week rotation inclusive of weekends: Two-week Rotation Mon/Tues/Sat/Sun 0800 - 2000 hrs Wed/Thurs/Fri 0800 - 2000 hrs We are looking for Community Support Workers (CSWs) to help us bring wrap-around patient-centered care to the Ajax/Pickering area. Competitive wage grid $20.96 to $23.20 per hour PAID MILEAGE at 0.45 per/km A special $500 to $750 bonus for signing with CarePartners Paid orientation and hands-on mentoring from experienced, caring staff Access to group discounts, including exclusive discounts from various retailers Total rewards program which includes health & dental benefits A robust Employee Assistance Program (EAP) for you and your family members Opportunities for continuing education and training Hands on mentoring and leadership support Opportunities to volunteer in countries with limited access to healthcare services What The Role Involves Contributing as a valued member of the interdisciplinary care team Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety Providing cognitive, social and/or emotional support to the patient Preparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practices Providing patient status reports and participate in patient/family care planning meetings What You Bring A strong desire and commitment to making a difference in the lives of their patients Exceptional interpersonal skills Ability to work both independently and as part of a team Experience working with patients with significant and complex care needs such as dementia and mental health A valid driver’s license and vehicle for which you are insured Community Support Worker (e.g.) Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or completion of first year Nursing program with preference given to those with a PSW certificate and/or equivalency To protect our patients, we require current vulnerable sector check screening, and a clear background check CarePartners In Your Community In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
