Job summary

Sagamok Anishnawbek is seeking a compassionate and organized Intake Case Management Worker to join the Mental Health and Addictions team within the Community Wellness Department.

This role serves as the first point of contact for individuals and families seeking services and supports. Through a comprehensive intake and case management process, the Intake Case Management Worker provides client-centred, strengths-based support that promotes recovery, stabilization, and independent living. Working as part of a multidisciplinary team, this position delivers individualized supports within the community and assists clients in navigating complex systems of care.

Position duties

• Client engagement and support
• Establish trusting relationships through consistent client contact and emotional support
• Conduct intake interviews, goal setting, and ongoing service planning with clients
• Support clients in developing engagement plans based on their recovery goals
• Help clients identify barriers and problem-solve solutions to achieve their goals
• Provide motivation and encouragement while celebrating client progress

• Case management and system navigation
• Provide information, referrals, and support in accessing mental health, physical health, social services, and justice-related systems
• Assist clients in accessing treatment, attending appointments, and arranging transportation
• Support clients in improving social determinants of health such as housing, employment, education, income supports, and disability benefits
• Advocate on behalf of clients to ensure access to eligible services and supports
• Provide crisis response and intervention as required
• Monitor client participation and progress, adapting supports as needed
• Maintain accurate and timely client records and documentation

• Collaboration and professional practice
• Work collaboratively with internal staff and external service providers
