We are looking for a Transitional Age Youth (TAY) Support Worker to join our team! Reporting to the Program Manager, Multi-Disciplinary Team, the Transitional Age Youth (TAY) Support Worker will bridge the gap between Child and Adolescent Mental Health Services (CAMHS) and Adult Mental Health Services (AMHS) by providing clinical interventions and supports, education, and advocacy to youth experiencing complex mental health and/or substance use challenges.
Permanent full-time, 37.5 hours per week. Note, this posting is for five active vacancies.
Provide mental health and addiction services and system navigation for transitional age youth (16-24) and their support networks Facilitate coordinated access to community supports that is developmentally appropriate, comprehensive, and flexibly adapted to the unique needs of youth Conduct needs assessments, develop and implement comprehensive care plans and ongoing monitoring until discharge Provide case management, task-based interventions and activities of daily living related to social determinants of health Assess and provide crisis support and plans, make connection to crisis services as appropriate Refer and link clients to community resources that support recovery goals and address social determinants of health Facilitate groups related to the social/recreational needs of youth and life skills, including development of new groups as needed Build on current and new partnerships to enhance coordination across the full continuum of community services resulting in positive outcomes, including avoiding unnecessary Emergency Department visits Advocate for clients to receive appropriate services Meet with other community service providers to share and collect information and best practices Update all appropriate documentation within 24 hours of client contact as required by CMHA PD policy and procedure

Post-Secondary diploma or degree in Social Services or degree in relevant discipline (BSW, MSW, OT, RN) Being a registered and / or regulated health professional is an asset Minimum 2 years of experience in transition or case management support Lived experience to support clients, validate their experiences, and support recovery Strong interpersonal skills with proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality. Willingness to continually improve understanding around cultural competence. Openness to operate from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework. Willingness to learn and apply practical understanding of different elements of diversity, such as gender identity/expression, race/cultural background, religion, sexual orientation, disability, immigration and precarious status, family status, etc. Fluency in a second language, including ASL (American Sign Language) is considered an asset. Valid Canadian driver’s license and access to an insured automobile in good repair. A satisfactory Vulnerable Sector Screening (Police Check) report.
