Job Summary The Care Coordinator is accountable and responsible for evaluating client needs, overseeing and monitoring the care delivered, developing an appropriate plan of care, and ensuring the competency of caregivers. The Care Coordinator commits to participate in establishing and implementing the highest standards of care.
The Care Coordinator is responsible for the quality and adequacy of services provided and will provide leadership and direction in all aspects of client care. She/he functions and participates in the development, implementation, monitoring, and evaluation of client programs and services while upholding Qualicare brand standards. This leader will assist in the delivery of home care services including client assessments, health education/counseling, and also will provide some direct client care.
