Overview

Assist in the planning and implementation of the individualized care plan for youth and their families. Facilitate social/recreational programs. Co-develop mental health and substance use supports with youth and the care team. Maintain youth in the community through regular and supportive contact with youth and their care team. Work in settings where contact with youth at risk can be established (e.g., street-based, school, and community settings). Promote Harm Reduction in substance use and collaborate with collateral service providers. Work under the general supervision of the Manager or designate as part of a multidisciplinary team

Overview

Provide nursing care to an assigned group of patients/residents according to established policies and procedures and the Standards of Practice for Licensed Practical Nurses of British Columbia. Function as an independent practitioner while accessing RN/RPN for collaborative support and/or clinical intervention when care needs fall outside LPN scope or when patient acuity increases. Assist patients/residents in meeting physical, social, spiritual, and psychological needs consistent with Northern Health mission, vision, values, and established protocols.
