Overview

Provide care to clients with substance abuse and co-occurring disorders through all phases of withdrawal management. Utilize a holistic approach for client assessments, planning, organizing, and implementing care. Deliver care in a variety of community settings including detox, stabilization, in-home, and outreach. Work in accordance with established standards, policies, and practices. Continuously improve quality and safety as a shared responsibility. Provide care to adults and youth with mental health and substance use issues. Facilitate groups and demonstrate leadership through teaching, coaching, and mentoring. Collaborate as part of an inter-disciplinary team and coordinate with internal/external contacts

Overview

Provide nursing care in accordance with provincial legislation, nursing standards, and the BC College of Nurses and Midwives code of ethics, as well as employer policies and competencies. Deliver client-centered care using a community-focused approach to promote, maintain, restore health, and prevent/treat/palliate illness and injury. Perform initial and ongoing client assessments (clinical and diagnostic reasoning) and provide nursing care through evidence-based guidelines. Collaborate with clients, the client’s health care team, and primary care providers to coordinate care. Plan and implement interventions and coordinate health services. Instruct clients and others on health care and well-being topics. Use relevant equipment and technology and complete documentation/communication to support inter-professional practice. Apply research and evidence-based best practices to professional nursing practice
