Act as a clinical resource in social work, education, and counseling for an identified population of patients with chronic diseases and/or barriers related to social determinants of health. Participate in case management. Provide assessment, care planning, intervention, and consultation when social, economic, emotional, or psychological circumstances present barriers to care or limit patients' function. Provide education and counseling using principles of adult learning.
