Overview
Deliver 1:1 supported independent living services to clients as part of the Psychiatric Outreach Program. Assist with planning and implementing psychosocial rehabilitation groups. Work closely with other community agencies to provide and coordinate services. Support clients in achieving treatment goals in collaboration with their clinician. Function as part of a multidisciplinary team in an outreach capacity. Support planning and carrying out of client care plans, including daily routines and ADL. Assess and monitor progress with treatment and support plans. Assist individuals with ADL and iADL. Adhere to established workflows. Provide outreach in clients’ homes and community settings, including travel within the community
