Overview
Provide outreach community-based support services as part of an ACT multidisciplinary, multi-agency outreach team. Assist in mental health and addictions treatment in accordance with an established plan of care/treatment. Provide life skills training, coaching, advocacy, and advice to clients experiencing mental health, behavioral, and developmental problems and to their caregivers/families. Motivate, assist, and instruct clients with activities of daily living. Report clients' progress and condition, including reactions to medications. Continuously improve quality and safety as a shared responsibility
