Job Description

The Case Manager provides social case management support as a primary care provider to the client. The population for this project are those who have difficulty engaging or have disengaged from care, involved in substance abuse/who are impacted by substance use disorder and Mental Health issues. Utilizing an outreach approach, the case manager provides client services including psychosocial assessments on intake, addiction assessments, client goal setting/skill building, and coordination/support of health/social services. A model of recovery services is utilized for those challenged with an addiction. The case manager acts as a client advocate while working with other health care professionals to provide services. The goal is to help clients establish a community based medical team and participate in collaborative planning, to achieve the best health possible, that may include stabilizing their mental Health and substance use disorders. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Candidates must hold a Bachelor of Social Work (BSW) and possess at least one year of experience in a health-related field. A valid Class 5 driver's license and registration with the Saskatchewan Association of Social Workers (SASW) are also required.
