Job Description

The Ontario Shores Centre for Mental Health Sciences is currently seeking a Registered Practical Nurse for the Hospital to Home Program (H2H). Reporting to the Clinical Manager for Complex Services, You will provide specialized care to patients with complex mental illness by using the principles rooted in the recovery focused model of care, built upon compassion, inspiration and hope.

The successful candidate will be a Registered Practical Nurse with sound clinical knowledge in the area of mental health and addictions. Our clients are living with severe and persistent mental illness and are in need of tertiary specialty services. The nurse will work actively with our clients and families, our multidisciplinary team, referral sources and community partners to ensure a smooth transition for a successful recovery-oriented discharge.

Requirements

  • Registration with the College of Nurses of Ontario in good standing
  • Current CPR – Health Care Provider (HCP) is required
  • 5 years psychiatric experience in a mental health facility, preferred
  • Demonstrated knowledge of College of Nurses of Ontario (CNO) Standards of Practice and commitment to maintaining reflective practice standards as described by the CNO.
  • Demonstrated knowledge of Canadian Standards for Psychiatric – Mental Health Nursing, and relevant policies and legislation, such as the Mental Health Act and PHIPA is required.
  • Proven ability to exercise mature, effective judgement and behavior in emergency situations.
  • The candidate will have demonstrated excellent interpersonal and effective oral/written communication skills including demonstrated excellent documentation practices.
  • Demonstrated excellent organizational, problem solving, decision-making, conflict resolution and critical thinking abilities
  • Implements and models the Recovery philosophy in all aspects of care as a Recovery Champion
  • Demonstrated in-depth knowledge and experience of the challenges of acute psychiatry
  • Proven skill and knowledge in the area of Psychosocial Rehabilitation and Recovery
  • Proven ability to work independently and within a multidisciplinary team
  • Understanding of recovery principles and their application to people with mental illness both in hospital and community
  • The ability to work with clients of diverse ethno racial and cultural backgrounds and proficiency in a second language would be considered an asset
  • Proven good attendance record with the capability of maintaining this same standard
  • Valid G class Drivers License

Travel is required for this position.
