Position Summary

The Clinical Coordinator is responsible for coordinating the clinical and operational delivery of outpatient services by supporting patient intake, referral management, scheduling, registration, billing, and care coordination activities. Working closely with physicians, clinicians, administrative staff, patients, and families, the Clinical Coordinator serves as the primary point of contact throughout the patient journey and facilitates seamless access to high-quality care. Utilizing advanced clinical knowledge and professional judgment, the Clinical Coordinator supports patient flow, triages referrals, coordinates interdisciplinary care plans, and ensures patients receive timely, effective, and patient-centred services. The incumbent contributes to quality improvement initiatives, operational efficiency, and financial stewardship while supporting UHN's vision of excellence in care, discovery, and learning.

Duties

Coordinate patient referrals, intake, triage, and service navigation. Review referral documentation for completeness and program eligibility. Schedule consultations, assessments, diagnostics, treatments, and follow-up appointments. Serve as the primary contact for patients, families, providers, and stakeholders. Support patient transitions throughout the care journey and discharge process. Monitor waitlists and facilitate timely access to care. Communicate appointment details, preparation requirements, and care information. Coordinate multidisciplinary case conferences and clinical consultations. Resolve routine patient and operational issues, escalating when necessary. Maintain positive relationships with patients, clinicians, physicians, and community partners. Coordinate patient flow, track patient progress, and address service delivery barriers. Support care planning for complex patients and collaborate with interdisciplinary teams. Maintain accurate patient registration, electronic records, databases, and documentation. Ensure compliance with privacy, confidentiality, information management, and quality standards. Process billing, invoicing, payment tracking, financial reporting, and related administrative activities.

Qualifications

Completion of a recognized degree in a regulated health profession such as Occupational Therapy, Physiotherapy, Nursing, Speech-Language Pathology, Social Work, Respiratory Therapy, Dietetics, or another related healthcare discipline. Current valid Certificate of Registration/Practice in good standing with the applicable Ontario regulatory College (e.g., COTO, CPO, CNO, CASLPO, OCSWSSW, CRTO, CDO, etc.) is required. Membership in the applicable professional association(s) is an asset. Minimum three (3) years of recent experience in a healthcare, rehabilitation, hospital, or outpatient clinical setting. Experience coordinating patient care, referrals, scheduling, intake, and service delivery processes. Experience supporting patients with complex health conditions and navigating multidisciplinary healthcare systems. Experience with billing, invoicing, accounts receivable, or financial administration activities is preferred. Experience using Epic or a comparable electronic health record system is strongly preferred. Experience participating in quality improvement, program development, or research initiatives is an asset. Strong understanding of outpatient clinic operations and patient flow management. Knowledge of healthcare funding and billing processes, including private-pay, third-party funding, and uninsured services. Demonstrated clinical judgment, problem-solving, and decision-making abilities. Proficiency with Microsoft Office Suite, including Outlook, Word, Excel, Teams, and SharePoint. Demonstrated commitment to patient-centred care, privacy, confidentiality, and professional practice standards.
