Salary Range

$33.80 to $42.25 Hours: 37.5 hr/week Shifts: Days/Nights Status: Temporary Full Time

Position Summary

As a member of the Gage Transitional team, the Community Service Coordinator promotes and educates the philosophy, purpose and objectives of the Gage TIL. Organize and coordinate information tours for prospective applicants. Organize and coordinate client bookings and support staffing. Provide information and expertise to staff and residents on community services/resources (specifically housing) available for the physically disabled. Assist in the planning, organizing, coordinating of a peer support system. Locate and influence the development of housing and support service options in the community. Complete timely and accurate clinical documentation. Be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors including: Compliance with the safety standards of the hospital and applicable legislation Identification and prevention of safety issues and problems Taking corrective action, where possible Attendance at safety meetings/training/updates in accordance with the Centre’s standards Ability to work in a manner that exemplifies the core values of UHN Evidence of good performance history and attendance record

Qualifications

Knowledge of and experience with individuals with physical disabilities Demonstrated leadership abilities in the following areas: program planning & development; coordinating internal & external resources; community liaison; education; training & support to varied professionals Bachelor’s degree in health science, social work or related field (minimum) Master’s degree in health science, social work or related field an asset Current registration with a Regulatory Health Professions College or equivalent Knowledge of community resources Strong interpersonal, organizational and communication skills (both written and verbal) Ability to work effectively within an interdisciplinary team Can work independently as well as with minimal supervision Ability to demonstrate a customer service philosophy Evidence of satisfactory attendance and punctuality
