The Licensed Practical Nurse provides nursing care for individuals and families with a broad range of care needs in their homes and other community locations. The Licensed Practical Nurse actively demonstrates a working knowledge of nursing techniques, principles, and practice in order to holistically care for the patient. Utilizes discretion to identify the need for Licensed Nurse support with patients due to acuity of care up to and including transfer of care to Licensed Nurse caseload.
Week 1: Wednesday, Thursday
Week 2: Monday, Tuesday, Saturday and Sunday
