Situated within identified primary care network locales, this position provides clinical pharmacy services to patients of the primary care network. As a member of an inter-professional team this position works directly with patients to identify and resolve actual and potential drug therapy problems through provision of comprehensive medication management. Travel may be a requirement of this position. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority in accordance with the service assignment and may require the use of a personal vehicle.
Licensed by the College of Pharmacists of British Columbia.
Two years of recent related patient care experience in a team-based primary care setting.
Valid Class 5 driver's licence.
Ability to perform patient assessments and therapeutic interventions.
Strong communication, critical thinking, problem-solving, and teamwork skills.
Computer skills including electronic health or medical records and Windows-based programs.
Physical ability to perform the duties of the position
