Job Title: Nurse Practitioner (NP) – Full Time
Responsible To: Executive Director
The NP will function in close collaboration with all physicians and Allied Health Care Professionals within the Family Health Team. As a vital member of the patient focused team, the NP will provide clinical services in collaboration with all health care professionals. In addition to providing a broad range of assessment, treatment, education and consultation services (specified by their scope of practice) the role of the NP will work with Family Health Team members to provide core services and specialized programs. The NP’s role also includes partnering with the Executive Director and other professionals in the development of programs, resources and the creation of linkages within the Family Health Team and community.
