Provide direct nutrition care services to patients of the Shapedown Program through assessment, planning, implementation, and evaluation of nutritional goals and care plans.
Conduct nutrition assessments and develop, document, implement, monitor, evaluate, and adjust age-appropriate nutrition care plans per Shapedown nutritional protocols.
Provide comprehensive nutrition counselling and deliver individual and group teaching sessions for program participants, including education for patients, parents, and caregivers.
Consult and communicate with community-based health professionals and other agencies to enhance continuity of care.
Contribute to planning, implementation, and evaluation of nutrition-related services as a member of the Shapedown team.
Document nutrition care plans and recommendations and maintain appropriate records (including workload statistics and patient databases).
Identify service needs and make appropriate referrals or develop alternate strategies for patient support, education, and follow-up care.
Provide input to program planning and review of treatment protocols/interventions.
Communicate and demonstrate nutrition care and Shapedown program principles to health care team members, colleagues, peers, and students via in-service education and training.
Participate in approved program research projects (surveys and outcome evaluation) including input into research questions, data collection, integration/interpretation, and implications for program delivery.
Participate in program quality improvement and risk management initiatives by collecting data, monitoring variances, recommending changes, and implementing approved changes.
Perform other related duties as assigned
Graduation from an academic program in dietetics approved by the College of Dietitians of British Columbia.
Full registration with the College of Dietitians of BC.
One year recent related experience in pediatric weight management and counselling (or equivalent combination of education, training, and experience).
Successful applicant must complete training required to become a certified Shapedown Weight Management Program provider.
Knowledge of pediatric obesity, disordered eating, and human metabolism.
Effective verbal and written communication skills.
Ability to work within an interdisciplinary team setting.
