Assist clients by providing support through day-to-day implementation of life skills and household routines.
Follow client care plan and deliver high quality client care.
Prepare meals and snacks according to the meal plan.
Use Catalyst system while assisting with medications on time according to Excel Society policies and procedures, including checking/receiving medication that arrive while on shift.
Perform general household duties and maintenance (cleaning, vacuuming, laundry) and outside maintenance (shovelling snow, cutting grass, gardening).
Maintain and update required documentation such as scorecards and medical records.
Assist clients with hygiene and personal care.
Provide emotional support and use de-escalation techniques.
Support clients in maintaining appointments, activities, or programs.
Monitor client wellbeing and physical/mental condition closely and report concerns to supervisor; accompany clients to appointments as necessary.
Liaise with families, guardians, professionals, and other members of the client support network and communicate relevant client information to team members
