About the role

Assist and support individuals in their daily routines and ensure quality care delivery.

Provide care within Health Care Competencies.

Implement the care plan in coordination with the Licensed Practical Nurse (LPN) and provide input when adjustments are required.

Refer clients and family members to appropriate health care team members when issues exceed the worker’s role boundaries.

Maintain respectful relationships and communication with clients and family while building rapport.

Help meet physical, mental, emotional, psychosocial, and cultural/spiritual needs of clients and provide support using active listening.

Support and encourage a culture of teamwork and communication.

Check equipment for defects, report defective equipment, and comply with infection control and isolation procedures.

Recognize preventative initiatives and follow guidelines for preventing fire and electrical accidents.

Become acquainted with clients, observe and report client status.

Encourage and support clients’ efforts to maintain/enhance health and wellness and assist with maintaining a clean, healthy, safe home environment.

Respect clients’ rights to confidentiality, privacy, independence, well-being, and choice.

Recognize and report signs of client abuse.

Recognize, respond to, document, and report altered client behaviours and complete daily shift client charting

Job

Responsibilities are as follows:</span></p> <ul> <li><span>&nbsp;</span>Providing care that is within the Health Care Competencies.</li> <li><span>Referring client and family members to appropriate members of the health care team for information about issues that exceed the boundaries of the role of the worker.</span></li> <li><span>Demonstrating the ability to have respectful relationships and communication with clients and family while building rapport.</span></li> <li><span>Helping to meet physical, mental, emotional, psychosocial, and cultural/spiritual needs of the client. Identifying potential sources, types, and signs of stress for clients and providing support as able including using active listening skills.</span></li> <li><span>Supporting and encouraging a culture of teamwork and communication.</span></li> <li><span>Checking equipment for defects, reporting defective equipment to supervisor or other designated individual, complying with infection control practices, and isolation procedures and precautions.</span></li> <li><span>Recognizing preventative initiatives and follow-up procedures of occupational hazards and follow guidelines for the prevention of fire and electrical accidents.</span></li> <li><span>Demonstrating ability to become acquainted with the clients and demonstrate awareness and understanding of client's needs. Observing and reporting client status.</span></li> <li><span>Encouraging and supporting client's efforts to maintain/enhance their health and wellness and assisting client with the maintenance of a clean, healthy, and safe home environment.</span></li> <li><span>Respecting client&rsquo;s rights to confidentiality, privacy, independence, well-being, and choice.</span></li> <li><span>Recognizing and reporting signs of client abuse.</span></li> <li><span>Recognizing, responding, documenting, and reporting altered client behaviours.
