About the role

Collaborate with the Wellness Manager to plan, implement, and oversee community member care and wellness services.

Conduct scheduled and change-of-condition wellness assessments.

Develop, implement, review, and evaluate individualized care plans.

Coordinate and lead care conferences with community members, families, and healthcare providers.

Maintain knowledge of wellness services and coordinate service delivery.

Liaise with physicians, hospitals, pharmacies, and external healthcare partners to ensure continuity of care.

Assist community members and families with health teaching, medical documentation, questions, concerns, and healthcare navigation.

Assist with assessments for prospective and existing community members, including move-in assessments when required.

Administer medications and ensure safe handling, storage, and documentation (including narcotics and controlled substances).

Receive, transcribe, and process prescriber orders in accordance with policy and professional standards.

Provide professional nursing care within scope of practice, care plans, and prescriber orders.

Support daily department operations and ensure appropriate utilization, maintenance, and storage of supplies and equipment.

Support vaccine clinics, health promotion initiatives, wellness education programs, and vital signs clinics.

Respond to community member emergencies and liaise with Emergency Medical Services when required.

Provide oversight and leadership to Unregulated Care Providers (UCPs) in accordance with regulatory and organizational requirements.

Maintain accurate, complete, and timely health records and documentation.

Conduct monthly, quarterly, and annual audits as directed.

Support compliance with the Retirement Homes Act, RHRA requirements, and V!VA policies and procedures.

Lead and participate in quality improvement initiatives, wellness programs, and committees (including Infection Prevention and Control and Falls Prevention).

Provide oversight and support for Infection Prevention and Control practices within the community.

Assist the Wellness Manager with achievement of departmental goals and objectives.

Participate in wellness meetings, community events, and daily Making Today Great meetings.

Support orientation, onboarding, training, and development of Wellness Team Members.

Provide coaching, mentorship, and support to Wellness Team Members and contribute to continuous learning.

Provide leadership and guidance to Wellness Team Members in the absence of the Wellness Manager.

Demonstrate V!VA Orange Dot Values in every interaction.

Foster positive relationships with community members, families, visitors, healthcare partners, and team members.

As a Wellness Coordinator (RPN) at V!VA Leaside, you will play a key role in coordinating and supporting the delivery of quality wellness services for our Community Members. Through your clinical expertise, compassion, and commitment to exceptional care, you will help ensure Community Members receive outstanding care and service while fostering a culture of accountability, collaboration, and continuous improvement. Reporting to the Wellness Manager, you will work closely with Community Members, families, healthcare professionals, and fellow Team Members to coordinate care, complete assessments, develop Care Plans, support regulatory compliance, and contribute to an exceptional Community Member experience. All care services are delivered in accordance with individualized Care Plans, V!VA policies, and applicable legislative and regulatory requirements. Key Responsibilities: Care Coordination & Clinical Excellence

Requirements

Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.

Minimum 3 years of nursing experience in retirement living, long-term care, seniors' care, or related healthcare environment.

Experience completing assessments and developing individualized care plans is an asset.

Experience working with older adults.

Strong clinical assessment and care planning skills.

Knowledge of Microsoft Office (Outlook, Word, Excel, Teams).

Strong communication, customer service, and interpersonal skills.

Ability to work independently while contributing to a collaborative team environment.

Ability to manage competing priorities and adapt to changing situations.

Commitment to person-centred care and continuous professional development.

Ability to maintain confidentiality and exercise sound professional judgment.

Ability to effectively communicate in verbal and written English.

Valid CPR/First Aid Certification required.

Smart Serve Certification is an asset.

Valid Vulnerable Sector Screening (VSS) required.

Ability to safely lift, carry, push, and move up to 50 lbs.

Ability to work flexible hours, weekends, and statutory holidays as required.

Available for call-ins and emergency coverage when required.
