Maintain and advance own clinical competence.

Plan, implement, evaluate, and coordinate patient access and care transition activities between Interior Health facilities and health care services across the continuum of care for assigned patients with complex care planning and health service needs.

Provide comprehensive assessment of patients; identify variances from care/discharge plans and service needs; resolve patient, caregiver, and system issues.

Establish and facilitate communication linkages between Interior Health facilities, health care services, community partners, physicians, health professionals, and patients and their families.

Communicate changes in day-to-day access and care transition service needs of patients

  • Graduation from an approved School of Nursing plus two (2) years recent related clinical experience which includes a combination of Community and Acute Care nursing experience working with the assigned complex patient/client population; or an equivalent combination of education, training and experience
  • Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) required
  • Current valid B.C. Drivers License required

Requirements

Graduation from an approved School of Nursing plus two (2) years recent related clinical experience including a combination of Community and Acute Care nursing experience with the assigned complex patient/client population; or an equivalent combination of education, training, and experience. Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) required. Current valid B.C. Drivers License required
