Provide ongoing intensive outreach nursing care, case management, and mental health and substance use support services for assigned clients in hospital, residential, or other community settings.

In accordance with the established mission, vision, values, and strategic priorities of Northern Health, and under the general supervision of Manager or designate, the Registered Nurse practices in accordance with the standards of professional practice and code of ethics as outlined by the BC College of Nurses and Midwives (BCCNM) as well as within a patient and family centred care model. The RN/RPN provides ongoing intensive outreach nursing care, case management and mental health and substance use support services for assigned clients, whether they are in hospital, residential or other community settings. Designs, implements, and monitors individual treatment plans to maximize community functioning and reduce re-hospitalization. Works in close association with inter-professional community mental health and substance use staff, emergency services, Indigenous partners, and community service providers. Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant.
Minimum 2 years nursing experience in mental health and substance use programming or an equivalent combination of education, training and experience.
Current valid BC drivers license. Skills and Abilities:
Ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning) and provide nursing care through appropriate / prescribed technical, therapeutic, safety type interventions.
