We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a Temporary to Full-Time opportunity in Buffalo Center, IA. In this role, you will provide essential chronic disease management, routine nursing assessments, and person-centered daily care for residents in a facility-based long-term care setting. You will work closely with RNs, CNAs, physicians, and therapists to maintain our residents' health, safety, and quality of life. Key Responsibilities Resident Care & Monitoring: Monitor vital signs, perform routine physical assessments, and report changes in condition to RNs, physicians, and families. Medication & Treatments: Safely administer medications, perform wound care, and execute dressing changes per individual care plans. Daily Support: Assist residents with activities of daily living (ADLs) and rehabilitation activities as directed.
Maintain clear, prompt, and accurate records in the Electronic Health Record (EHR) while adhering to facility safety protocols.
Team
Collaboration: Partner with a multidisciplinary team to implement, refine, and deliver comprehensive care plans.
Education: Graduate of an approved Practical Nursing program.
Active LPN license in the state of Iowa (or a valid compact state license).
Current Basic Life Support (BLS) / CPR certification.
Strong communication skills with residents, families, and healthcare team members.
1+ years of experience in a long-term care or skilled nursing facility. Hands-on experience with EHR documentation and care planning. Demonstrated knowledge of wound care or palliative care principles. Work Environment & Schedule Facility-based long-term care setting. Rotating shifts in a collaborative team environment alongside nurses, CNAs, and therapists.
