About the role

The Social Worker provides therapeutic services, resident assessments, and care planning for residents and families within a long-term care setting. They also act as a liaison between families, community partners, and the interdisciplinary team to ensure resident rights and quality care standards are met.

Job Description

Applications are invited for the position of full time Social Worker to work at Extendicare Cedars Villa our 243 bed Long-Term Care Home located in SW Calgary, Alberta. Reporting to the Executive Director, the Social Worker provides social work services for residents and families within legislative and budgetary parameters.

Responsibilities

Participates in resident admission process including tours, admission, transfers, discharge, and occupancy tracking. Acts as liaison with families and advocate for residents to ensure they are allowed and encouraged to exercise their rights Participates in the interdisciplinary team process of resident assessment, care planning and evaluation to ensure services are planned according to individual resident and family needs. Provides therapeutic services through individual and group counseling for residents and families, documents on resident’s record and evaluates effectiveness of these programmes. Collaborates with Psychogeriatric Consultant in support of staff caring for residents. Adheres to established department plans, policies, procedures, and quality assurance programme to meet all regulatory and Extendicare standards. Provides in-service programs for home staff, residents, and families. Encourages and supports professional growth and development of staff. Liaises with educational institutions and provides supervision to social work students during field practicum. Promotes other quality initiatives and programmes designed to enhance resident care. Maintains all required records, reports, statistics and documentation for both residents and department in accordance with policies, procedures, and legislative requirements. Participates on committees related to resident care and operation of the home as required. Liaises and consults with community partners, company resources and professionals in relation to resident care needs and department activities. Assists and Liaises with Government departments to obtain benefits on behalf of the Resident and Public Trustee and Guardian. Other duties as assigned by the Administrator

Qualifications

University Graduate in Social Work, Mandatory. Registration with the Alberta College of Social Workers Mandatory. Previous experience in a long-term care setting preferred. Excellent communicator, well organized, team player with solid skills in assessments and care planning, supportive therapy, crisis intervention and grief counseling. Expertise in working with Senior populations with Alzheimer’s disease, dementia, Mental Health, responsive behaviors and other psychogeriatric issues, preferred. Knowledge of applicable Long Term Care legislation and standards, preferred. Knowledge of occupational health and safety practices, principles and legislation, preferred. Computer literate with understanding of RAI-MDS and PointClickCare software system, preferred. Requirements

Candidates must hold a university degree in Social Work and be registered with the Alberta College of Social Workers. Previous experience in a long-term care setting and expertise in psychogeriatric issues are preferred.
